IMPORTANT NOTE: Graduation at St. Augustine

The University does not guarantee that arrangements can be made for your presentation if you do not return this completed form to the Examinations Section, St. Augustine by September 28th, 2012. If you wish to be presented at the Mona, Cave Hill or Open Campus graduation, return one (1) copy to the Campus at which you wish to be presented and immediately contact the Senior Assistant Registrar (Examinations) at the Campus for Campus-specific instructions. This is most important as the arrangements at each Campus differ.
YOU MUST HOWEVER COMPLETE THE FORM FROM YOUR CAMPUS.
