Instructions for Nomination

1. Nomination forms are available by clicking the Nominate tab above and selecting Nomination Forms under the respective category (Employee Excellence Awards). Click the link to download the relevant form;
2. Applicants can self-nominate or be nominated by members of the University of the West Indies, St. Augustine campus;
3. Please read form in entirety before commencing nomination;
4. Please fill out nomination form completely. (All fields are required);
5. Persons nominating are required to complete the nomination form in full and provide the requisite evidentiary requirements (as outlined in the rubric) to satisfy the criteria for assessment (incomplete nomination forms or inadequate evidence submission will result in the non-acceptance of the application);
6. After forms have been completed in its entirety and relevant evidentiary documents attached they must then be submitted to EE/SE Secretariat for consideration. This can be done by attaching and submitting the completed form via email to eoyseawards@sta.uwi.edu.
7. Nominees must consent to any nominations submitted on their behalf. As such, the Human Resource Department (HR) will verify consent to all nominations. Illegitimate nominations will not be considered;
8. Nominators must also sign the nomination form before submission;
9. Nomination forms must be submitted no later than the deadline date given which is Wednesday 18th November 2015;
10. Once the deadline has passed no nominations can be made and therefore the nomination process would be considered closed;
11. Awards will be given to successful nominees at an awards ceremony on  Friday December 18th 2015;
12. Awardees are to receive their awards in person. If unavailable, they are to notify the Secretariat of a suitable proxy at least three (3) days before the Awards Ceremony;
13. No awards will be granted posthumously;
14. Awardees must be available for individual and group photography for marketing purposes;
