Application for Provisional Entry to Examination Venue

The Provisional Entry to Examination Venue Form

A student whose course registration is not completed by publication of the official examination timetable is required to complete this Form to obtain provisional entry to the examination venue. After completing this Form, and following payment of tuition and late fees, please return receipt with Form to the Students and Receivables Section, Bursary for processing. After financial approval is obtained, please have the Form approved by Student Affairs, Admissions OR Graduate Studies and Research.

STUDENTS ARE ADVISED THAT ENTRY TO EXAMINATION VENUES WILL BE DENIED IF REGISTRATION FOR THE COURSE CANNOT BE CONFIRMED.

Your valid UWI Identification Card must be presented in order to gain entry into the examination venue. Candidates shall bring their UWI Student identification cards to each examination. They must display their identification cards in a prominent position on their desks. Candidates shall not be allowed to write an examination without a valid UWI Identification Card. [Examination Regulations 2011/2012 90(i)] If you have not yet received your ID card, please visit the ID Card Office at the Ground Floor, DAAGA Lecture Room 1. STUDENTS ARE ADVISED THAT ENTRY TO EXAMINATION VENUES WILL BE DENIED IF VALID UWI ID IS NOT PRESENTED.

If you have not yet received financial clearance, please liaise with Students and Receivables, Bursary to regularize your account.
STUDENTS ARE ADVISED THAT ENTRY TO EXAMINATION VENUES WILL BE DENIED IF FINANCIAL CLEARANCE CANNOT BE CONFIRMED.

NOTE: It may take 48 hours for your registration record to be reinstated.

Please ensure that the signed and stamped copy of the Provisional Entry to Examination Venue Form is presented at the examination venue to gain entry.
